Sunnyvale, California-based Intuitive Surgical, Inc. (ISRG) develops, manufactures, and markets products that enable physicians and healthcare providers to enhance the quality of and access to minimally invasive care. Valued at $140.5 billion by market cap, the company offers endoscopes, endoscopic retractors and disectors, scissors, scalpels, forceps, needle holders, electrocautery, ultrasonic cutters, and accessories during surgical procedures.
Shares of this global technology leader in robotic-assisted minimally invasive surgery have underperformed the broader market over the past year. ISRG has declined 22.1% over this time frame, while the broader S&P 500 Index ($SPX) has rallied nearly 19.5%. In 2026, ISRG stock is down 33.9%, compared to SPX’s 11.6% rise on a YTD basis.
Narrowing the focus, ISRG’s underperformance looks less pronounced compared to the State Street SPDR S&P Health Care Equipment ETF (XHE). The exchange-traded fund has gained about 17.4% over the past year. Moreover, the ETF’s 8.1% returns on a YTD basis outshine ISRG’s double-digit losses over the same time frame.
Despite reporting solid underlying revenue and procedure volume growth, ISRG has underperformed over the past year primarily due to tight hospital capital budgets and emerging competition from rival robotic systems. Investors are particularly concerned that changes to Affordable Care Act (ACA) exchange coverage could weigh on elective procedure volumes. These regulatory shifts have forced ISRG into a conservative posture, signaling a potential moderation in procedure-driven instrument demand over the near term.
For the current fiscal year, ending in December, analysts expect ISRG’s EPS to grow 22.9% to $8.87 on a diluted basis. The company’s earnings surprise history is impressive. It beat the consensus estimate in each of the last four quarters.
Among the 31 analysts covering ISRG stock, the consensus is a “Strong Buy.” That’s based on 22 “Strong Buy” ratings, two “Moderate Buys,” six “Holds,” and one “Strong Sell.”
This configuration is more bullish than a month ag, with a “Moderate Buy” rating overall, consisting 20 analysts suggesting a “Strong Buy.”
On Aug. 13, Joanne Wuensch from Citigroup Inc. (C) maintained a “Buy” rating on ISRG, with a price target of $500, implying a potential upside of 33.5% from current levels.
The mean price target of $483.34 represents a 29.1% premium to ISRG’s current price levels. The Street-high price target of $685 suggests an ambitious upside potential of 82.9%.
